NDIS Support Coordination

Support Coordination is an NDIS-funded support aimed to help you make the most of your NDIS funding package. Specialist Support Coordination is comparable to Support Coordination, but it is financed in cases where the demands are particularly severe or complex.

This assistance will help you develop the skills needed to understand, implement, and apply your plan. A support coordinator will collaborate with you to ensure that a variety of supports are used to enhance your ability to maintain connections, manage service delivery duties, live more independently, and participate in your community.

Three Levels of Support Coordination

The NDIS defines three levels of Support Coordination:

  • Level 1: Support connection helps participants connect with informal community support so that they can maximize their NDIS plans.
  • Level 2: Support coordination helps participants understand and utilize their NDIS plans.
  • Level 3: Specialist support coordination designed for participants who require specialist support due to higher levels of complexity. Involves assistance from a specialist support coordinator.
What We Can Offer

The support coordination that we provide includes, but is not limited to the following:

  • Find and assess supports and services
  • Negotiate services and their prices, develop service agreements and create service bookings
  • Help build a participant’s capacity to act independently and exercise choice and control
  • Arrange service assessments
  • Budget for each support type
  • Liaise with plan managers
  • Resolve issues, problems and conflicts with your providers as they arise
  • Coordinate service delivery where you have multiple providers
  • Make sure services are provided in accordance with your needs and preferences
  • Understand your responsibilities under service agreement
  • Managing reports
  • Assess whether you achieved your goals and received value for money for your plan
  • Identify solutions to problems experienced in implementing the plan
  • Consider new goals
  • Initiate service agreement review
  • Visit you at home or at a place convenient to you to discuss support coordination of your plan
